Job Description:
<div>The Perinatal Nurse Navigator is an experienced obstetrical nurse who focuses on positively impacting perinatal care, infant mortality, and morbidity in eastern North Carolina by providing quality patient care to high-risk perinatal patients.&nbsp; The nurse navigator provides individualized assistance to specific patients and obstetrical practices by coordinating virtual care (telemedicine) with Maternal-Fetal Medicine specialists.&nbsp; The nurse navigator will facilitate timely access to care, coordinate referrals, and provide education and support.&nbsp; The nurse navigator will work directly with the practices providing the virtual care and will collaborate closely with the maternal-fetal medicine service.<br>&nbsp;</div><div>1.&nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; Patient assessment and identification of high-risk perinatal patient needs: Identify barriers and help overcome these barriers, locate resources, coordinate services, and referrals, and provide education on diagnosis, risk factors, childbirth and plan of care in collaboration with the multidisciplinary team, obstetricians/maternal-fetal medicine and the practices collaborating on telemedicine.<br><br>2.&nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; Communication: Serve as primary contact and liaison with obstetrical practices, Maternal-Fetal Medicine and the patient:&nbsp; build and maintain the trust of the patient and family; provide emotional support; navigate the health care system using knowledge of intricacies of each system and serving as the patient advocate assuring timely access.&nbsp; Serve as a communication conduit between obstetricians/maternal-fetal medicine service and the obstetrical care providers in the referral region.&nbsp; Coordinate provider feedback to regional providers who have referred their patients.<br><br></div><div>3.&nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; Resource:&nbsp; Provide patient, family, and staff education related to pregnancy, high-risk perinatal care, newborn care, postpartum care as well as postpartum mood and anxiety disorders, and contraceptive options.&nbsp; Maintain a comprehensive database of resources for patients and families including financial, psychosocial, education, emotional, transportation, and cultural.<br><br>4.&nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; Collaboration with the medical team: Create and maintain open lines of communication with physicians and their support staff- in collaboration with obstetricians/maternal-fetal medicine establish referral pathways to decrease the delay in receiving high-risk perinatal care and consultation. &nbsp; Travel to each obstetrical practice participating in the telemedicine program to increase referrals and improve relationships.<br><br></div><div>5.&nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; Quality:&nbsp; Maintain accurate and complete documentation, ongoing assessment of role in decreasing disparities in care. Participate in quality initiative implementation such as Perinatal Quality Collaborative of North Carolina, Baby-Friendly Hospital, Long-acting reversible contraception, and others to improve quality outcomes of patients in eastern North Carolina, therefore, improving infant mortality and morbidity.&nbsp; Maintains knowledge of the latest research in the care of perinatal patients.<br><br>CLINICAL SUPPORT POSITION - NOT SUBJECT TO STATE HUMAN RESOURCES ACT.<br><br>The ECU Brody School of Medicine is affiliated with Vidant Medical Center.&nbsp; Vidant Medical Center is the flagship hospital for Vidant Health and serves as the teaching hospital for the Brody School of Medicine at East Carolina University.</div><div><br></div>

Minimum Qualifications:
<div>Graduation from state-accredited school of registered nursing and two years of nursing experience, with at least one year in the assigned specialty area. Must have a current license to practice as a Registered Nurse in North Carolina by the North Carolina Board of Nursing. All degrees must be received from appropriately accredited institutions.</div>
